Medical Records
All medical records and clinical consultations are strictly confidential. Records cannot be released without the written consent of the patient except in the event of a life-threatening medical emergency, in response to a subpoena, or for reporting purposes as required by law.
MyChart proxy access allows users to access the medical records of family members and others they care for with the patient’s permission. Cadets can grant access by filling out a form or with a proxy invite through the Friends and Family Access page in MyChart.

To send medical records from another facility to MUSC Health at The Citadel, download and complete the form below. Email the completed form with a copy of the patient’s state or government-issued ID to citadel@musc.edu for processing.
